General Office Policies
There is a 48-hour cancellation policy. You must call the
office 48hours prior to your appointment to cancel or reschedule.
Otherwise, you will be charged for the full cost of your visit.
If there are extenuating circumstances please provide this information.
Please notify Mary of any changes to your address, telephone numbers.
When leaving messages please leave both home and work numbers. Most calls are returned in the evening after business hours.
For After-Hour Emergencies
For a true clinical emergency
do not wait for a return call, go directly to the nearest hospital
emergency room or call 911.
Release of Client Information
Except for the purpose of medical/psychiatric emergency or
court subpoena, records and information about treatment are released
only with the written permission of the client or parent/legal guardian.
You will receive HIPAA compliant privacy information at your first
session.
